This form is for Teachers, Students, or District Personnel requesting funding distribution from the Millwood Enrichment Foundation. Please ensure the request has been reviewed and approved by the appropriate district administration before submission. Requests not reviewed by district personnel, as well as incomplete applications, will not be considered.
